Okay, voting is now open. I redid the form structure into a spiffy new wizard format, so it shouldn't be as ugly. Just head to the main page of the site (click the header logo anywhere), and you should see the form. I did this from scratch in an hour this morning so let me know if it doesn't work.

I'll be getting mobile working shortly, but the mobile format may be cumbersome. Who knows.
